Acredia’s software now fully supports the Aged Care Transfer Summary (ACTS), streamlining the digital handover of critical health information when residents transition from aged care to a hospital setting. ACTS standardises this process, ensuring essential details like medications, health summaries, and transfer reasons are efficiently shared, improving continuity of care.
Aged care providers are welcome to register their interest in piloting this functionality via our system.
This enhancement directly aligns with recommendations 66 and 68 from the Royal Commission into Aged Care Quality and Safety, reinforcing best practices in aged care communication and patient safety.
Seamless Digital Transfer of Critical Health Information
Residential aged care facilities can now:
- Upload transfer details, including the reason and date of transfer, automatically generating a Residential Care Transfer Overview in My Health Record.
- Share comprehensive health data, including the Residential Care Health Summary and Residential Care Medication Chart, either simultaneously or at a later stage.
- Ensure care continuity, as the Transfer Overview links to existing clinical documents in the resident’s My Health Record, such as:
- The latest shared health summary
- Discharge summaries
- Advance care planning documents and directives
- Immunisation record
Enhanced Accessibility for Healthcare Professionals
If other healthcare providers involved in a resident’s care are connected to My Health Record, they can seamlessly access the Residential Care Transfer Overview, ensuring they have up-to-date, accurate health information. This integration enhances coordination, reduces duplication, and supports better health outcomes for aged care residents.
