Acredia Software Integration with My Health Record

As Australia’s population ages, the need for seamless, connected healthcare for older Australians has never been more critical. At the heart of this mission lies My Health Record, the national digital health platform overseen by the Australian Digital Health Agency (ADHA), enabling better information sharing, continuity of care, and real-time connectivity across healthcare settings. 

Acredia is pleased to participate in this transformation, assisting our customers to deliver better-connected and continuous care.  Our integration with My Health Record now supports the uploading of the new Aged Care Transfer Summary (ACTS) documents, and in early 2025, it will support vaccination history and discharge summaries.

Acredia’s Commitment to My Health Record Integration

Acredia’s clinical and medication management systems are designed to align with the ADHA’s national digital health strategy. Our integration with My Health Record ensures aged care facilities have access to up-to-date health information for their residents, improving decision-making and facilitating smoother transitions between care settings.

By working with the ADHA, Acredia’s integration with My Health Record allows for authorised resident health information to be accessible to everyone involved in their care – from aged care staff to healthcare professionals and family members.

 

Approval Under ACIO 1

Acredia achieved My Health Record conformance during the ADHA’s first Aged Care Industry Offer (ACIO 1) from 2021 to 2023. This milestone marked our systems’ integration with the Healthcare Identifiers Service, enabling aged care facilities to connect to My Health Record effectively.

Progress in uptake has been made since the introduction of ACIO 1, with 37% of aged care facilities registering for My Health Record as of 30 June 2024

 

ACIO 2 and the Aged Care Transfer Summary (ACTS)

Building on this success, the second Aged Care Industry Offer (ACIO 2) introduces advanced capabilities, including the Aged Care Transfer Summary (ACTS) document (now available). Acredia is pleased to be an approved vendor participating in ACIO 2 and is expected to achieve full conformance early in 2025.

The ACTS document simplifies and standardises the digital handover of key health information when a resident transitions from aged care to a hospital setting. This tool directly addresses recommendations 66 and 68 from the Royal Commission into Aged Care Quality and Safety, ensuring critical health details like medications, health summaries, and transfer reasons are efficiently shared.

In addition, Acredia will also support additional key health information in My Health Record, including:

  • Vaccination records: From the Australian Immunisation Register (AIR), including COVID-19 vaccinations, vaccine details, and dose history.
  • Discharge summaries: Ensuring that hospital-to-home transitions are smooth and all care providers can access comprehensive post-hospitalisation information.
